The Quality Assurance & Performance Improvement (QAPI) Specialist is an individual th will be assisting with agency efforts toward the process of improving clinical documention and outcomes by taking a systemic, comprehensive, and da-drive approach to maintaining and improving safety and quality. The QAPI Specialist compiles trends and reports quality da in pient care, safety, risk management, infection control, outcomes and customer sisfaction and performs and uses audit results to lead development of Performance Improvement Plans. The QAPI Specialist has a variety of responsibilities including development of process improvement plans, record reviews, da tracking and analysis, and serves as a regulory resource for clinical staff. To be successful in this role, you must have a passion for quality clinical care and enjoy sharing knowledge with others.

Promotes broad understanding, communicion, and collaborion with team members to ensure approprie communicion, integrion, and utilizion of best practices.

Participes in the development, implemention, and evaluion of Clinical Quality improvement action plans to improve quality.

Participes in Agency's QAPI activities, including but not limited to, pient incidents, employee incidents, infection tracking, and quality indicors such as OASIS (Outcome and Assessment Informion Set) and pient sisfaction.

Continually assesses clinical staff compliance as required by internal policies and external standards of performance.

Assists and coordines with comprehensive quality improvement, da analysis and reporting

Prospectively reviews OASIS assessments to ensure approprieness, completeness and compliance with federal and ste regulions and organizion policy.

Ensures approprie ICD-10 coding and sequencing as it reles to the pients medical condition, including any co-morbidity.

Develop interventions to impact quality scores

Participes in deleged groups and providers on improving quality scores

Reviews corpore indicors da and other comparive da.

Promotes performance improvement plans th are consistent with cultural diversity, healthcare literacy issues and language.

Performs clinical record audits and provides educional feedback to staff.

Facilites the use of evidence-based guidelines, criteria, and other clinical tools to reduce variion in clinical practice and to optimize clinical outcomes.

Assists agency meeting all requirements, ensuring compliance in all applicable settings, and managing reliable processes for timely and accure submissions of required da to the proper payors and regulory bodies.

Qualificions

Associe of Science Degree-required

Bachelors degree in nursing, PT, OT, SLP, MSW, Health Administrion, Management, or reled discipline-preferred

Experience

3 years of certified home health experience required

Value Based Purchasing, PDGM, CAHPS-preferred

Licensure, Certificion, Registrion

CT Licensure in field of specialty

Licensure, Certificion, Registrion

Experience in providing da analysis and stistical processes in quality reporting

Ability to design and develop performance measures

Experience in providing educion

Experience in facilition of PI teams using quality improvement methodology

Ability to effectively communice both verbally and in writing

Ability and willingness to travel to branches (regions) as needed

Travel within the Unites Stes may be necessary

Knowledge of federal and ste regulory requirements

Excellent written and verbal communicion skills
