Position PurposeThis role drives quality improvement, regulatory compliance, and clinical education across homecare and/or hospice agencies. It partners with agency leadership to design, implement, and evaluate performance improvement initiatives , while ensuring adherence to EMR standards, accreditation requirements, and documentation accuracy .
The position also serves as a clinical educator and mentor , leading onboarding, training programs, and competency development for staff.
Key Responsibilities Quality & Performance Improvement- Lead QAPI (Quality Assurance Performance Improvement) initiatives
- Analyze outcomes data (OASIS, OBQI/OBQM, EMR reports) to drive improvements
- Develop action plans to achieve top-decile clinical and patient satisfaction outcomes
- Conduct audits, chart reviews, and compliance monitoring
- Ensure compliance with CMS, state, and accreditation standards
- Support ADR audits, payer reviews, and ALJ preparation
- Maintain survey readiness and oversee documentation accuracy
- Identify trends impacting billing, compliance, and reimbursement
- Design and deliver training for clinical and non-clinical staff
- Lead orientation, onboarding, and competency validation
- Provide ongoing education in documentation, EMR workflows, and regulations
- Apply adult learning principles to training strategies
- Review and validate OASIS and EMR documentation
- Support discharge workflows and case conferencing
- Collaborate with leadership on policy development and process improvement
- Assist with student clinical programs and interdisciplinary initiatives
- Partner with agency leadership to achieve clinical, operational, and financial goals
- Act as a subject matter expert and mentor
- Lead or support programs such as:
- Infection prevention
- Patient satisfaction
- Safety initiatives
- Drive change management and continuous improvement
- Licensed RN or Therapist
- Bachelors degree
- 4+ years experience in homecare or hospice
- Strong knowledge of:
- Quality improvement methodologies
- Regulatory standards
- EMR systems and documentation
- 2+ years in a supervisory or leadership role
- Experience with committees or organizational leadership
- Strong communication and interpersonal skills
- Ability to adapt in fast-changing environments
- Valid drivers license and reliable transportation
- OASIS Certification (within 15 months for homecare roles)
